How to go about doing this in my book?

I'm working on the basic idea and plot of a new novel I'm planning. In it, the main character and two others are transported to an island with no memory at all, besides simple things-- how to talk/walk/eat, they also know about the objects of the real world, boats and cars and things. the point is that they don't remember their names.

what i want is for their names to be tattooed either on their wrists or the backs of their necks. should the names be their real names from the real world, or should they be new names, with some sort of significance?

Cool idea. I think the names should not be their real names, but something important to them in their previous life. And they should be random things like a zodiac sign, a rock band, a television or movie reference, a Biblical reference, a short saying, an emblem, etc. Examples: Capricorn, Metallica, Genesis 1:1, Semper Fi, Live Long and Prosper, Army Strong, a butterfly, a heart, a gator, etc.

also, what is a good book cover for the book?

I love that book! And the movie is amazing too! Its cliche, but its such a good love story. A good book cover would be a picture of a butterfly, because that is the kind of tattoo she gets.
